Scrip Order Form
Scrip is ordered the 2nd Tuesday of each month, for pick-up the following Sunday.  Orders are due in the office by the Monday before the Scrip is ordered.

Facilities Use Request Form

Everyone (including First Church groups) wishing to use First Church facilities must complete the Facilities Use Request Form and turn it in to the office by mail, fax (630-629-2610) or by hand for approval.  You will be notified when your request is approved.  Requests for dates for September 2008 and later may be turned in at any time, but will not be confirmed until March 2008.  Church activities take priority in initial scheduling. 

This form is not for wedding requests.  Please contact the church office for information and avaibility of the Maple Street Chapel for weddings.
